Your first visit to 5 Leaf Wellness establishes a vital foundation for our relationship with you. During your first visit, we make sure to obtain important background information about your medical history and give you time to get to know your doctor.

Things to know before you come to your appointment:
No insurance is accepted, this visit is completely self pay.
Cost: $300 for initial visit if you pay cash; $315 if you pay with a debit or credit card; Follow up visits are $100 cash or $105 with a card.
There is a $77.50 fee that is paid with a credit card online to the State of Florida for the application once a year. It take the State of Florida 10-14 days to process and approve the application. Once you receive your email approval then you will be able to go to the dispensory.
You will need to bring a valid Florida Driver's License and an electronic device that can access your email with you to the appointment.
Follow-up appointments are every 105 days. Should you need to reschedule your orders expire the night before your original appointment.
